General Terms and Conditions

To ensure a smooth and successful function, we seek your acceptance of our standard policies and procedure. Please check each box and initial as listed:

DEPOSIT(Required)

This deposit will be refunded provided that the entire facility room rented, public
restrooms, and lobby is restored to its original condition.
Patron reserves the rights to a walk-thru following the event (available only Monday
through Friday). Should the event end past 6 PM, a walk thru will take place the
following day. Damage to the premises shall be at the expense of the user and must be
paid within 30 days of the event date (money will be taken from security deposit. If
damages exceed the deposit, patron must make payment of remaining balance within the
30 day time frame). The patron shall indemnify and hold harmless the Greater Los
Angeles Agency on Deafness, Inc., and employees from any such losses, damages, and/or
claims.

The Greater Los Angeles Agency on Deafness, Inc. Requires that all Patrons be
responsible for any damage done to the premises during their event of by any outside
entity hired for the event by them. Patron shall be responsible for payment (payments for
damages will be paid in full to GLAD) of any and all damages to the buildings,
furnishings, fixtures, or equipment whether caused by the User or their guests. In the
event that damage is done from an event GLAD reserves the right to select the contractor
to do the repair.

PAYMENT(Required)

PAYMENT

Deposit of 50% is required for all functions to secure space on a definite basis. The deposit is due one month prior to event. Final balances are due one week prior to the event. All payments are to be made by credit card of cashier’s check. In the event of a cancellation, notice must be given 14 days prior to event for 50% refund. Should advance notice not be provided, the deposit will serve as the cancellation fee. GLAD shall have the right and absolute discretion to cancel this agreement in the event of a major catastrophe on or related to the premises including but not limited to floods, fires, earthquakes, and such. If, for any reason, this cancellation occurs, the deposit will be fully refunded. NO JUMPING CASTLES ARE ALLOWED INSIDE UNDER ANY CIRCUMSTANCES. JUMPING CASTLES ARE ALLOWED OUTSIDE ONLY. MUST DISCUSS OUTSIDE PLACEMENT OF JUMPING CASTLES WITH GLAD REPRESENTATIVE AHEAD OF TIME. NO PAINTING.

A cleaning fee is available for all major events held on our premises

LIABILITY(Required)

LIABILITY

The Greater Los Angeles Agency on Deafness, Inc. Cannot assume responsibility for the
damage or loss of any merchandise or articles on the premises prior to, during, or
following your event. GLAD is not liable for actions pre-during-post event. Evidence of
liability coverage must be provided before the event. Patron is responsible for his/her
own insurance liability coverage.

PUBLIC SAFETY(Required)

Renter agrees to conduct its activities upon the premises so as not to endanger any person
thereon; and to indemnify, defend and hold harmless The Greater Los Angeles Agency
on Deafness, Inc from all demands, claims, suits, actions, or liabilities resulting from
injuries or death to any person caused during the event. An adult must supervise
children at all times. All furniture is to stay indoors. Business offices are off limits.
An automatic fine of $100.00 will be charged for setting off any fire alarms or
tampering with fire extinguishers. Fire escapes are off limits, with the exception of
emergencies.

AREAS OFF LIMITS(Required)

The third floor and office spaces are off limits at all times. Do not disturb residents for
any reason. Please be considerate and aware of the fact that this is a place or residency
and business.

PARKING(Required)

PARKING

Double parking is not permitted. Handicap rules apply. Main parking lot is off limits for
it is designated for residents and Chief Executive Officer (please follow posted signs).
Side lots on both Laverna and Norwalk are to be utilized along the street parking. Please
do not block any of the neighboring driveways.

NOISE(Required)

NOISE

At all times, please keep the noise level to a minimum. The noise limit is 10 PM for all
events occurring in the evening.

ALCOHOL / FOOD(Required)

ALCOHOL / FOOD

Alcohol consumption is allowed on the premises ONLY for people over the age of 21.
You shall assume full responsibility for any liability of your guests that may result from
the consumption of alcohol while on the premises of The Greater Los Angeles Agency on
Deafness, Inc. Should patron decide to sell a license must be obtained. The alcohol
license, which may be obtained from the State of California Department of Alcohol
Beverage Control, must be provided to GLAD prior to the start of the event. The sale of
alcohol is prohibited unless a seller’s permit is obtained. All food is to be kept in the
dining room. Fires or cooking inside are not permitted.

DECOR(Required)

All decorations and exhibits must be flameproof to meet the first prevention regulations as stated in the Los Angeles City Fire Prevention Code Manual and any other applicable codes. Open flames will not be allowed. Candles are permitted with votives one inch over top of flame. The Patron is expected to comply with all applicable laws and obtain all necessary permits. Smoke machines are not permitted on our premises, due to the wiring of our smoke detectors.

GLAD will not permit the affixing of anything to the walls, floors or ceilings with nails, staples, or hot glue. Decorations may be displayed with tape with advanced approval. Use of confetti, glitter, birdseed, and rice are not permitted. Deliver/pickup of items shall be on the day of event due to our storage limitations. Prep time, if needed, must be arranged with GLAD representative.

In accordance with California State Law, smoking is not permitted anywhere, in any location indoors of GLAD. Designated smoking are outdoors 20 feet from the building.

GLAD staff is not employed by any other organization. Therefore they are not able to assist other organizations that utilize our facility. Staff is not available to move furniture, perform clerical duties, serve refreshments, or complete any other tasks.(Required)

GLAD requires on-site security guard(s) to monitor each event with 50+ people in attendance. Patron is solely responsible for paying for one or more security guards. Contact GLAD representative if you need contact information for suggested security companies. Any events with 100+ attendants are required to have an additional security officer present (totaling 2) provided by GLAD. This is a mandatory rule that will be enforced at all events. Event is not to exceed stated room capacity.
